In Islamabad, Rawalpindi and throughout Pakistan, many men mistakenly think that an enlarged man’s breasts are just their chest fat. However, there are two separate conditions: true gynecomastia versus chest fat (pseudogynecomastia) and two different treatments for each. This is the difference so you can identify your condition correctly and choose the right path forward.
Quick Facts
- True gynecomastia = excess glandular breast tissue, which may be firm or rubbery, as a result of hormonal imbalance.
- Pseudogynecomastia (chest fat) = soft fatty tissue from weight gain, decreases when diet and exercise;
- Gynecomastia does not shrink with weight loss alone in most cases
- Surgical correction usually needs liposuction, gland excision, or both

True Gynecomastia
True gynecomastia refers to the abnormal enlargement of the glandular tissue of the breast in male, typically due to an imbalance between estrogens and androgens. It can happen as part of puberty, with some medications, when taking steroids, liver or thyroid disease or just as we get older. This is a structural tissue, unlike fat, which cannot be lost with exercise or diet.
Chest Fat (Pseudogynecomastia)
Pseudogynecomastia, or chest fat, is the term used for the excess of fat (adipose tissue) deposits found in the chest region, typically associated with the increase of body weight or body fat percentage. This is not a hormonal disorder, but rather a condition that can be corrected by a regular diet and exercise routine.
True Gynecomastia Vs Fat: What Is The Difference
Texture and Feel
- True Gynecomastia: Firm and rubbery and often a lump under the nipple
- Chest Fat: Like other parts of the body, it’s soft and spongy.
Pain and Tenderness
- True Gynecomastia: Painful, particularly around the areola.
- Chest Fat: Typically it is pain-free.
Shape
- True Gynecomastia: Rounder, more defined, can mimic female breast shape
- Chest Fat: Flat and hanging, evenly distributed around chest area
Response to Weight Loss
- True Gynecomastia: Minimal change or may be more apparent following fat loss
- Chest Fat: Decreases in the wake of continued weight control
Why Is the Difference Important For Treatment?
The first step to the correct treatment plan is proper diagnosis. Simple fat is not a medical issue and it is not necessary to have surgery for this condition, but dealing with glandular gynecomastia by diet alone is rarely successful.
Treating Chest Fat
- Structured cardio and strength training
- Calorie-controlled diet
- Consistency over a period of months
- Non-invasive fat reduction in select cases
Treating True Gynecomastia
- Gynecomastia surgery (male breast reduction) combining liposuction and glandular excision
- Under sedation and/or general anesthesia
- Discreet incisions around the areola to minimize scarring that can be seen.
- Stable lifestyle post surgery, with lasting results.
Treatment Procedure for Gynecomastia Surgery
At Royal Cosmetic Surgery Clinic, the procedure used to reduce the size of the male breast is as follows:
- Initial Consultation: Physical exam and, if needed, hormonal/blood work to confirm true glandular gynecomastia vs fat
- Pre Surgery Planning: Surgeon determines if lipo, excision or both should be performed
- Anesthesia: Procedure performed under either local sedation or general anesthesia depending on the complexity of the case
- Liposuction Step: Excess fat is removed by using a thin cannula inserted into small incisions.
- Excision Step: The excess glandular tissue and skin are excised and removed through a discrete, peri-areolar incision.
- Contouring: Chest is sculptured for a flatter and more masculine chest profile
- Closure: Minimally scarring incisions, dressing applied.
- Recovery Room: Patient is monitored for a short period of time prior to a same day discharge.
- Follow up Care: Taught to wear compression garments and follow up with visits to monitor healing
- Final Results: Final chest contour after 2 to 3 months when swelling has gone down.
After Gynecomastia Surgery Do’s and Don’ts
| Do’s | Don’ts |
| Wear the prescribed compression garment | Don’t skip or remove the garment early |
| Light walks to support blood circulation | Don’t do heavy cardio or for at least 4 weeks |
| Follow the post-op medication and | Don’t miss your follow-up appointments |
| Eat a balance good diet to support healing | Don’t smoke or consume alcohol during recovery |
| Sleep on your back | Don’t sleep on your stomach or side |
| Keep incision sites clean and dry | Don’t expose the chest to direct sun |
| Maintain a stable, healthy weight long-term | Don’t gain weight or use steroids |
If you are experiencing tenderness or a firm lump, these are often the first signs of gynecomastia that you should not ignore.
How Islamabad Patients Get Diagnosed Correctly?
These patients from Islamabad, Rawalpindi and other parts of Pakistan at Royal Cosmetic Surgery Clinic undergo a hands-on physical examination and if required hormonal testing to determine if the chest tissue is glandular, fatty or a combination of both (mixed gynecomastia). Only excision or only liposuction or a combination is suggested depending on this clinical difference.
